Clubs should ensure that all players aged 14 years and over registering to play for the club are supplied with the Cardiac Screening Questionnaire. This should be completed by the player (under the supervision of an adult if under 18), and any issues should be raised with their GP.
Clubs are responsible for the supply fo the Questionnaire, but the completion of it is the responsibility of the player (and parent / guardian).
